GİRİŞ ve AMAÇ: Tiroidektomi sonrasında vokal kord bakısında remifentanil infüzyonunun hemodinamik yanıt ve hasta konforuna etkisini araştırdık.
YÖNTEM ve GEREÇLER: ASA I-II, 18-60 yaş, 40 hasta çalışmaya dahil edildi. Anestezi indüksiyonu için tüm hastalara; 1.5 microgr/kg fentanil, 5-7 mg/kg tiopental, 0,1 mg/kg vecuronium uygulandı. Cormack- Lehane (CL) skoru kaydedildi. Anestezi idamesi % 50 O2-N2O ve % 1 sevofluran ile sağlandı. Grup R’de (n=20) Remifentanil 0.05-0.5 microgr/kg/dk idamede uygulandı. Grup K’da (n=20) %0.9 NaCl 5 ml/sa infüzyonu yapıldı. İnhalasyon anestetiği operasyon sonunda kesildi. Grup R’de; spontan solunumu inhibe etmeyecek şekilde sedasyon dozu Remifentanil 0.02 microgr/kg/dk, Grup K’da; %0.9 NaCl 5 ml/sa infüzyonu; operasyon sonrasında laringoskopi değerlendirmesi sırasında devam etti. Hemodinamik parametreler ve postoperatif Cormack- Lehane skoru kaydedildi.
BULGULAR: Grup K’da; CL skoru grup R’den anlamlı olarak yüksek bulundu (p=0.015). Postoperatif laringoskopide kalp hızı grup K’da anlamlı olarak yüksek bulundu. grup K’da 6 hasta Postoperatif Laringoskopiyi hatırlıyordu (p=0.018). Hemodinamik parametrelerde gruplar arasında fark bulunmadı.
TARTIŞMA ve SONUÇ: Remifentanil tiroidektomi sonrasındaki laringoskopi prosedürü için uygun bir ajandır.
INTRODUCTION: We studied the effects of remifentanyl infusion on hemodynamic response and patient comfort; during the inspection of the vocal cords after thyroidectomy.
METHODS: ASA I-II, aged 18- 60 yr, 40 patients were included study. All patients were given 1.5 μgkg-1 fentanyl, 5-7 mg/kg-1 thiopental, 0,1 mgkg-1 vecuronium for anaesthesia induction. Cormack-Lehane (CL) scores were recorded. Anaesthetic maintenance was ensured through 50 % O2-N2O and 1 % sevoflurane. In group R (n=20) Remifentanyl 0.05-0.5 μg kg-1 min–1 infusion were used maintenance. In group K (n=20) 0.9 % NaCl 5 mLh-1 infusion was used. Inhalation anaesthetic agents were discontinued by the end of the operation. In group R; Remifentanyl 0.02 μg kg-1 min –1, which was sedation dose that didn’t due to respiratory insufficiency and in group C 0.9 % NaCl 5 mLh-1 infusions were contuined after the surgery during the postsurgical evaluation laringoscopy. Haemodynamic parameters and postoperative Cormack- Lehane scores were recorded again.
RESULTS: In group K; CL scores were significantly higher than the other group. (p=0.015) Postoperative laringoscopy heart rate was found significantly higher in group K. (p=0.001) 6 patients in group C (30 %) remembered the postoperative laringoscopy prosedure (p=0.018). There were no statistically differences between groups regarding the hemodynamic parameters.
DISCUSSION AND CONCLUSION: Remifentanyl is suitable agent for the patients in the procedure of laringoscopy following thyroidectomy.
