The Effect of Different Suturing Techniques In Penetrating Keratoplasty On Postoperative Astigmatism And Visual Outcome of Keratoconus Patients

INTRODUCTION: To compare the results in keratoconus patients who underwent penetrating keratoplasty (PK) using interrupted or single continuous suturing techniques in terms of postoperative astigmatism and visual outcome.
METHODS: Penetrating keratoplasties were performed in 61 eyes of 61 keratoconus patients between January 2012-December 2014. Single continuous running suture tecnique was used in 30 patients (group 1), and interrupted suture technique in 31 patients (group 2). The patients who had no comorbidities, who had no increase in intraocular pressure that may affect the transparency of the graft cornea after PK, who did not experience graft rejection, graft ulceration or graft detachment, and who tracked their follow-ups were included in the study. Patients’ age, gender, autorefractometeric measurements and best corrected visual acuity (with Snellen chart) were compared in these two groups. The autorefractometeric results were measured before the surgery, 1 month 3, 6, 12 months after surgery, and 3
months after removing sutures.
RESULTS: There was no statistically significant difference between the two groups in terms of preoperative astigmatism (P=0.217>0,001). There was no statistically significant difference between the mean astigmatisma values after the removal of the sutures, as well (P=0.1).
Preoperative mean visual acuity was 0.07±0.05 in group 1, and 0.1±0.06 in group 2. The preoperative visual acuity was statistically significantly lower in group 1 than group 2 (p=0.046<0.05), whereas there were no statistically significant difference between the groups in the final visual acuities after the removal of the sutures(P=0,217 >0,05).
DISCUSSION AND CONCLUSION: There is no significant difference between the interrupted and single continuous suturing techniques in terms of the refractive values and the visual outcome. We think that the choice of suturing technique is up to surgeon’s experience.

Penetran Keratoplasti Uygulanan Keratokonus Hastalarında Farklı Sütür Tekniklerinin Ameliyat Sonrası Astigmatizma ve Görsel Prognoz Üzerine Etkisi

GİRİŞ ve AMAÇ: Keratokonus hastalarında tek tek ve tek devamlı sütür teknikleri kullanılarak yapılan penetran keratoplasti (PK) sonuçlarını ameliyat sonrası astigmatizma ve görsel sonuç yönünden karşılaştırmak.
YÖNTEM ve GEREÇLER: Ocak 2012-Aralık 2014 arasında 61 keratokonus hastasının 61 gözüne PK uygulandı. Bu hastaların 30’unda devamlı (grup 1), 31’inde tek tek kesintili (grup 2) sütür tekniği kullanıldı. Kontrol muayenelerine düzenli gelen, keratokonus dışında ek patolojisi bulunmayan, keratoplasti sonrası greft saydamlığını etkileyebilecek göz içi basıncı (GİB) artışı olmayan, greft rejeksiyonu, greft ülseri, greft ayrışması geçirmemiş olan hastalar çalışmaya dahil edildi. Her iki grupta yer alan hastaların yaşı, cinsiyeti, ameliyat öncesi, ameliyat sonrası 1.ay, 3. ay, 6. ay, 12. ay ve sütürler alındıktan sonra 3. ayda yapılan otorefraktometre ölçümleri (sferik ve silindirik değerleri) ve en iyi düzeltilmiş görme keskinlikleri (Snellen eşeli ile) karşılaştırıldı.
BULGULAR: Ameliyat öncesi astigmatizma değerleri açısından iki grup arasında istatistiksel olarak anlamlı fark bulunmadı (P=0,217>0,001). Sütürler alındıktan sonra ortalama astigmatizma değeri karşılaştırıldığında da istatistiksel olarak anlamlı bir fark görülmedi (P=0,1). Ameliyat öncesi ortalama görme keskinliği grup 1’de 0,07±0.05, grup 2’de 0,1±0.06 düzeylerindeydi. Grup 1’de ameliyat öncesi görme keskinliği grup 2’ye göre istatistiksel olarak anlamlı düzeyde düşük bulunurken (p=0.046<0.05) sütürler alındıktan sonra final görme keskinliklerinin iki grup arasında istatistiksel olarak anlamlı bir fark yaratacak seviyede olmadığı saptandı (P=0,217 >0,05).
TARTIŞMA ve SONUÇ: PK’da uygulanan tek devamlı sütür ve tek tek sütür tekniklerinin refraktif değerler ve görsel sonuçlar açısından farkı yoktur. Sütür tekniği seçiminin cerrahın tecrübesine bağlı olduğunu düşünmekteyiz.
