Performance of the ‘Turkish Triage System’ During the Covid-19 Pandemic

INTRODUCTION: In Türkiye, a triage system based on a 3-color code (red: very urgent, yellow: urgent, green: non-urgent) is mandatory in all public hospitals. This study aimed to analyze the triage codes assigned to confirmed COVID-19 patients according to their outcomes to test the reliability of the triage system. Our study provides data on our triage system, whose reliability has not been tested before.
METHODS: Patients with positive PCR tests who were admitted to the emergency department between March 15 and December 31, 2020, were included in the study. Assigned triage codes and outcomes were analyzed retrospectively.
RESULTS: Of the 7040 patients, 4176 (59.3%) were coded as ‘green’ and 333 (4.7%) were coded as ‘red’ at triage. The sensitivity of green triage for recovery was 60.29%, and the specificity was 78.78%. The sensitivity of red triage for critical outcomes and mortality was 57.14%, and the specificity was 96.59%. Being over 58 years old, regardless of gender, was a predictive factor for critical outcomes and mortality.
DISCUSSION AND CONCLUSION: Based on this study, it is possible to state that the ‘Turkish Triage System’ is successful in recognizing patients in critical situations, but its performance in distinguishing cases with recovery is moderate in the COVID-19 patient group.

COVID-19 Pandemisinde ‘Türk Triaj Sistemi’nin Performansı

GİRİŞ ve AMAÇ: Türkiye’de, 3 renkli kodlamaya dayalı bir triaj sistemi (kırmızı: çok acil, sarı: acil, yeşil: acil değil) tüm kamu hastanelerinde zorunlu olarak kullanılmaktadır. Bu çalışma, triaj sisteminin güvenilirliğini test etmek amacıyla doğrulanmış COVID-19 hastalarına verilen triaj kodlarını hasta sonuçlarına göre analiz etmeyi hedeflemektedir. Çalışmamız, daha önce güvenilirliği değerlendirilmemiş olan triaj sistemimize ilişkin veriler sunmaktadır.
YÖNTEM ve GEREÇLER: 15 Mart–31 Aralık 2020 tarihleri arasında acil servise başvuran ve PCR testi pozitif olan hastalar çalışmaya dâhil edildi. Verilen triaj kodları ve hasta sonuçları retrospektif olarak analiz edildi.
BULGULAR: Toplam 7040 hastanın 4176’sı (%59,3) triajda “yeşil”, 333’ü (%4,7) ise “kırmızı” olarak kodlandı. İyileşmeyi öngörmede yeşil triajın duyarlılığı %60,29, özgüllüğü ise %78,78 idi. Kritik sonuçları ve mortaliteyi öngörmede kırmızı triajın duyarlılığı %57,14, özgüllüğü ise %96,59 olarak bulundu. Cinsiyetten bağımsız olarak 58 yaşın üzerinde olmak, kritik sonuçlar ve mortalite için prediktif bir faktör olarak saptandı.
TARTIŞMA ve SONUÇ: Bu çalışma ile “Türk Triaj Sistemi”nin kritik durumdaki hastaları tanımada başarılı olduğu; ancak COVID-19 hasta grubunda iyileşmeyle sonuçlanan olguları ayırt etmedeki performansının orta düzeyde olduğu söylenebilir.
